The HCC program expanded to include
20 comprehensive health centers across
Jordan.

• 5231 patients benefited from various

activities that have been implemented
within the HCC program in 2018 and 224
MoH staff were trained on the HCC program
and materials.

• Regular blood tests were conducted

for each participant according to their
condition in order to follow up on their
health status, including BMI, HbA1c, lipid
profile, fasting blood sugar, TSH and blood
pressure.

• 9 key interviews were conducted with the

heads of the health centers, and 10 focus
group discussions were conducted with a
representative sample of the beneficiaries
of the HCC program in four health centers,
to assess the impact of the program on the
beneficiaries and the community.
The focus group results showed that the
beneficiaries satisfaction with the high
level of services and activities of the HCC,
and that resulted in improvements in their
health status because they acquired the
ability to manage their chronic diseases
through a healthier lifestyle.
